Attention CHCs and PCAs: 
Innovation Award Opportunity!

The Kresge Foundation has authorized NCB Capital Impact to issue a request for proposals (RFP) that will result in four or more innovation awards of up to $25,000 each that will be given to CHCs and PCAs in recognition of their work in advancing innovations in the delivery of primary care, addressing the social determinants of health, and building healthy communities. All U.S. based CHCs and PCAs are eligible.

Applications must be completed no later than no later than close of business, 5:00 p.m. Pacific Time, July 20, 2012. Final awardees will be notified by August 10, 2012.
